REFUND POLICY:
Refunds for any of the above pre-registered Courses or Workshops (less $50) provided only by cancellation request received in writing (email or letter) at least one week prior the scheduled start date of Course or Workshop.
For $25 additional fee, any prepaid registration to a Course or Workshop can be applied to another Course or Workshop offered at a future date if request is received in writing (email or letter) at least one week prior to original scheduled start date of preregistered Course or Workshop.
ABSOLUTELY NO refunds given AFTER one (1) week prior to schedule start date of Course or Workshop unless participant's reservation can be filled and paid by another.
Instructions for Check Payments:
1) Print a copy of this form.
2) Fill out this form.
3) Make your check made payable to Linda Greenfield.
4) Mail to: Linda Greenfield, 9420 SW 77 Ave, Darlow BLDG, Suite 201, Miami, FL 33156
Other information:
Linda personally processes all credit card payments through Square. Square will e-mail to you a receipt for your payment. You will also receive a printed receipt directly from Linda for your payment at the first class.
Directions to the class site and miscellaneous information will emailed to you approximately four (4) days prior to the first class date.
